At the same time, our structured model helps referral agencies, hospitals, probation and parole officers, grant reviewers, and community partners understand how the program supports positive outcomes.

Our vision is to cultivate peaceful, supportive communities where individuals experiencing life's challenges have the opportunity to stabilize, develop practical skills, strengthen resilience, and transition toward independent living with confidence and purpose.
Our mission is to walk beside residents as they rebuild their lives through the AAS 5-Level Integration Readiness System, guided by NARR Standards 3.0, nursing best practices, and Rhode Island BHDDH principles.
HOS provides a structured, faith-centered environment, peer support, wellness coaching, and individualized guidance that empowers residents to move from stabilization toward lasting independence.
